You will be asked to provide your personal information and to select the services you would like ICES to provide. Then you will identify the purpose of your evaluation and provide us with a brief history of your education.
When finished, you will be asked to print our application and to follow the mailing instructions provided. Before you start the application, please review the documentation requirements below.
