ICES can issue additional copies of the evaluation report we have previously prepared for you. This service is available up to 10 years after the report was first issued. If your report was prepared more than 10 years ago, you will have to apply and pay for a new report. To order copies of a previously prepared evaluation report, please fill out the form below and click Continue. ICES reserves the right to deny a request for a copy of an evaluation report after the report has been prepared.

The fees are $30 for the first copy and $10 for each additional copy ordered at the same time. Unofficial fax copies are $10 each. Copies will be issued within ten business days of receiving your request and the required fees.
